Question 4 and 5 are specifically asking for details of the equipment supplied by their ISP and is assuming that they are separate ADSL Modem and Router/Access Point, but there will be many people who do not get supplied with the equipment, and have to buy their own equipment, and also may have a combined ADSL Modem/Router/Access Point

I am one of those people, and I use a Draytek Vigor 2800VG, which is a combined Wireless Router with the ADSL Modem built in

More importantly, a lot of customers in the US use cable modems, which don't use PPPoE at all, making most of the questions inapplicable. Non-routing DSL modems (which don't ever expose PPPoE to the user) are also very common, and aren't represented in the survey either.

Indeed, rather incomplete list of options to choose from. I use a cablerouter (UTP to coax) and have no detailed information on what exactly is used to connect to the ISP infra. The cablerouter uses DHCP to provide an IP address (on the internet) to my LAN/WAN Router (WNDR3700).

It also doesn't mention PPPoA

In the UK, most connection are PPPoA

A very narrow scoped survey, I must say. For example, my ISP does not use ADSL/PPPoE. The Internet connection is provided via CAT5E cable and the wireless router it is connected to belongs to me.

Amen to that.
The info provided by the survey is so slim that I did not bother to fill it in.
I will though, when the survey is more relevant ( ... my provider's name? what's up with that?)
